The Stanislaus Health Foundation Board of Directors is a working Board. We are a fundraising organization that attracts key members in our community with a vested interest in helping all the people in Stanislaus County… and beyond.

- Initiative. This Community Health Improvement Plan focuses on:
- Four Social Determinants of Health:Access to Health – information, resources and health care for all residents of Stanislaus County
- Basic Needs – access to sufficient and affordable healthy food
- Education – the opportunity to attain a high school diploma or equivalent to attend college or vocational training
- Built Environment– infrastructure that supports the health and well-being of our residents
